Why does the state show no reports, and are individual records maintained?
What the records show
The six-year reporting gap is documented. Whether it reflects non-submission, routing, an identity mismatch or an unincorporated correction is unresolved. Aggregate reporting and maintaining individual immunization histories are separate duties.
What this does not establish
Missing aggregate reports are not proof that individual records are missing. No school response about this gap has been obtained.
What needs to happen next
- Request DHS and local-health-department receipt logs, late or corrected submissions, notices and follow-up correspondence for school code 3619/1530.
- Ask the school to explain the gap and confirm, in aggregate, how it maintains required histories and waivers. Do not request identifiable student health records.
- Reconcile the records with both agencies before assigning responsibility. Correct or close the finding if the published data are wrong.
